What California requires
Medical records
California physicians must retain medical records for at least seven years after the last date of service. Records for minor patients must be kept until at least one year after the patient turns 18, and providers billing Medi-Cal face a stricter ten-year requirement. Licensed providers who cease operation must still preserve records for seven years following patient discharge.

Court transcripts
California requires transcripts used as the official record to be certified by a licensed Certified Shorthand Reporter (CSR). A licensee must list their CSR number on both the transcript cover page and the reporter certificate, which is how a court or opposing counsel verifies a transcript is genuine. Since 1 January 2023, reporters transcripts must be delivered in electronic form unless a party entitled to the transcript requests paper. Formatting is prescribed: lines are consecutively numbered and double- or one-and-a-half-spaced.
