What Texas requires

Medical records

Texas physicians must retain medical records for at least seven years from the date of last treatment, or longer where other federal or state law requires it. For patients under 18, records must be kept until the patient reaches 21 or for seven years from last treatment, whichever is longer. Records connected to a civil, criminal, or administrative proceeding may only be destroyed once the physician knows the matter has been finally resolved.
